• You are classified as a full-time or part-time non-union employee.
• Are a Non-Union employee or Legacy Qwest Union Represented Retail/Outside Sales Representative, (as defined in the Glossary, of a Participating Company (see "Non-Union employee"), (please refer to the SPD below).
• Are a Non-Union employee or Legacy Qwest Union Represented Retail/Outside Sales Representative who is full-time and whose standard hours are 30 or more hours per week.
• Have completed the one (1) year service requirement and satisfy all other Plan eligibility requirements (please refer to the SPD below).
