It doesn’t matter if you are working on your physical wellness, financial wellness, or another area, Lumen's wellness program is designed to help you live a balanced and optimal life. The Well Connected program provides access to a number of resources and activities to support your optimal health and performance.
REMINDER: The Rally Health Survey and Biometric Screening are both required to be completed by December 31st of each year to redeem any earned wellness rewards. Register online through RALLY.
Get well connected with Rally
Lumen's wellness program, Well Connected, helps you take charge of your overall wellbeing with various resources. The Well Connected program can improve your wellbeing and, through Rally, you and your eligible spouse/partner can each earn up to $600 in Well Connected rewards each year. Rally makes it easy to find what you may need, all in one place. Use Rally to view and connect to your health benefits, get personal health and well-being recommendations, and earn and track rewards.
Be preventive
Take charge of your health with preventive health screenings such as annual wellness visits and biometric screenings. These annual screenings are at no cost to you, provide a Rally reward, and can help you detect health risks before it’s too late. Also, depending on your age and gender, you may be due for a mammogram, colon screening or cervical cancer screening which also provides Rally rewards. Other ways to be preventive include flu shots and other vaccinations found in the Preventive Care Guidelines.
Be active
Be Active and get part of your fitness expenses reimbursed for things like fitness membership fees, classes, subscriptions, and activities. Employees and eligible spouses can also save money with the Active & Fit Direct gym discount program which is open to all employees and spouses. Don't miss out on all the fitness activities through Rally such as Walking Challenges, Rally Stride, and Rally Missions. Be active and save money at the same time!

Improving and maintaining chronic health conditions is a journey, and Lumen offers numerous programs and resources to help you on your journey no matter which diagnosis you have received. Be an active spouse
Eligible spouses/domestic partners can participate and earn rewards through Rally just like employees. Participate with your spouse/domestic partner, earn double the rewards, and get healthy together. Learn how to enroll, which activities spouses are eligible for, and how your spouse can receive their Rally rewards. Get support from your spouse/domestic partner to live a healthier life and make better choices, but make sure to have fun while doing it.

Lumen offers weight loss programs and resources for those looking to make healthier choices and lose weight. Join a program that fits your life and will help you be more successful in your weight loss journey. You can enroll in Real Appeal, join Rally missions, and get setup with a personal coach. We also offer the Total Health Immersion program to employees who medically qualify.
Be resilient - thriving through challenges
Resilience is arguably the most valuable skill to have in life especially in a world where employees are often connected to work 24/7. It’s now more important than ever to be more resilient. Lumen has increased the resources available for resiliency and mental health. We offer the Employee Assistance Program (EAP), Rally Missions, Coaching, and counseling services with Cor Medical clinics in specific locations to help you tackle the ups and downs of life.

Get your Flu shot! You can avoid illness by getting the flu vaccine at no cost to you. To help you stay healthy, Lumen is offering the Walgreens Voucher Program and the Retail Vaccination Program.
Maintain a healthy pregnancy
Pregnancy can be an exciting time of your life, but it can also bring on a variety of emotions and stress along the way. Lumen offers a variety of programs for expecting moms to keep you and baby healthy for all nine months. Two great programs offered include the Healthy Pregnancy Program and Pacify. Our benefits for mothers-to-be don't stop there, some benefits extend past delivery such as Milk Stork and dedicated nursing mother's rooms.
Be tobacco free
Being tobacco free is an important step in preventing long-term health risks. Lumen provides several tobacco cessation resources such as coaching and Rally Missions at no cost to you to help employees and eligible dependents quit the habit. Spouses/domestic partners and dependents age 18+ who are enrolled in a Lumen medical plan can participate in the Quit for Life tobacco cessation program.
